Team Document Submission
Hello. Here is where you will submit your player information, certified state roster,
and travel permit to Heartland Soccer. Here are the items you will need to fill out and
submit the forms on the next page:

Team # & Team Name
- The team number and the team name as shown in the final seeding on
Team Contacts & Seedings.

Your Email
- The email of the person submitting the form.

Certified State Roster
- This is your state roster that comes with your player cards when you get them from your club
registrar.

Travel Permit
- Except for MYSA teams, permits are only required if your team
is not
carded by the state of Kansas. *Obtainable through
